Pharmacies and pharmacy professionals must maintain current federal, state, and local licenses to legally operate and handle medications—especially controlled substances. Keeping licenses accurate, up to date, and well-organized is essential for compliance, audit readiness, and uninterrupted operations. 

This guide outlines the key areas of pharmacy licensing and registration management, including requirements for the pharmacy itself, controlled substance authorization, and individual staff credentials. 

 
 

DEA Registrations  

Any pharmacy that handles controlled substances must obtain and maintain a valid DEA registration. 

  • Initial Registration 
    Pharmacies must first obtain the appropriate state license and, if required, a state-controlled substances registration before applying for DEA registration. New registrations are completed using DEA Form 224.  
  • Renewals 
    DEA registrations must be renewed every three years using the DEA renewal application (Form 224a). Renewals can be submitted online up to 60 days before the expiration date.  
  • Important Compliance Note 
    If a DEA registration expires without renewal, the pharmacy is no longer authorized to handle controlled substances until it is reinstated.  
  • Ongoing Monitoring 
    Expiration dates should be reviewed regularly to ensure timely renewal. During renewal, registration details—such as pharmacy name, address, and DEA number—should be verified for accuracy, along with any required endorsements (e.g., authorized collector status).  
  • Updates & Display 
    Any changes to registration information must be submitted through the DEA’s modification process. Current DEA registration documentation should be properly displayed and filed according to regulations.  

 
 

State Pharmacy Licenses & Registrations 

Each pharmacy must comply with licensing requirements set by its State Board of Pharmacy. 

  • Required Licenses 
    Pharmacies must maintain an active state pharmacy license and, where applicable, a state-controlled substances registration.  
  • Verification & Accuracy 
    Licenses should be reviewed for:  
    • Correct pharmacy name and address  
    • Required endorsements (e.g., dispensing authority, pharmacist-in-charge designation, technician utilization plans, authorized collector status)  
    • Any additional state-specific requirements  
  • Renewals 
    State licenses must be renewed according to the timelines and procedures established by the state board. Renewal periods vary by state.  
  • Recordkeeping 
    Current licenses should be displayed and stored in compliance with state and federal regulations.  

 
 

Employee Licenses & Certifications 

Pharmacy staff are individually responsible for maintaining active professional licenses and required certifications. 

  • Common Credentials 
    Staff licensing may include:  
    • Pharmacist licenses  
    • Pharmacy technician or provisional licenses  
    • CPR certification  
    • Immunization certification  
  • License Accuracy & Endorsements 
    Staff should ensure:  
    • Required endorsements are active (e.g., immunization authority, preceptor status, PDMP registration)  
    • Personal information (such as address) is accurate if required  
    • Employer details are updated where applicable  
    • All state-specific requirements are met  
  • Renewals 
    Licenses and certifications must be renewed within designated timeframes set by the state board or certifying body.  
  • Ongoing Review & Updates 
    Licensure should be reviewed periodically to ensure compliance. Updated credentials must also be shared with relevant systems, vendors, and platforms when renewed or when new staff are onboarded.
